What's the best quality management software for healthcare in 2026?
Pick your QMS by who audits you: an accreditation surveyor, the FDA, or a notified body.
Incident reports land in a shared inbox. CAPAs live in a spreadsheet nobody owns. Or you build a medical device and your ISO 13485 auditor wants the approval trail for every SOP. Healthcare quality management software (QMS) puts reports, corrective actions, documents and training records in one place you can show an auditor.
We compared the tools that real healthcare organizations use. Short answer: Qualio if you're a young medtech that needs to be audit-ready fast, MedTrainer if you run clinics or a hospital and want incidents, policies and training together, SimplerQMS if you want validation included in a published price.

How we compared and scored
This page is for two people: the quality or risk manager at a hospital, surgery center or clinic group, and the QA/RA lead at a company that makes a medical device or a drug. Different jobs, same core need: log a problem, fix it, prove it's fixed.
A tool only made the ranking if its official site shows both things: real healthcare or life sciences customers (a healthcare industry page, named customers or healthcare regulations), and a workflow for reporting issues with corrective actions. We read pricing and feature pages plan by plan, then user reviews, starting with the negative ones.
Weights follow what makes a healthcare QMS work. Incidents and CAPA count for 25%: that's why you buy one. Fit for your regulator (accreditation, ISO 13485, Part 11, validation) counts for 25%, because that's what the auditor checks. Real cost and price transparency count for 20%. Documents and audits count for 15%, time to value for 15%.
A hidden price lowers the cost score but doesn't knock a tool out by itself. Most healthcare vendors publish nothing, and dropping them all would leave you with two options.
- Incidents and CAPAIncident or nonconformance reporting, root cause analysis, CAPA (corrective and preventive action), effectiveness checks.25%
- Fit for your regulatorAccreditation readiness for providers; ISO 13485, FDA QMSR, Part 11 e-signatures and validation for manufacturers.25%
- Real cost and transparencyPublished price or not, separate implementation fees, free read-only seats.20%
- Documents and auditsDocument control with versions and approvals, internal audits, training records.15%
- Time to valueRollout time, configuration effort, validation workload, ease of use in reviews.15%

Qualio
Qualio says it works with life science companies "of all shapes and sizes", and its plans are built for teams that are just getting audit-ready. Foundation covers document control, training, supplier quality, design controls, five quality event templates and three compliance frameworks (FDA, MDR, ISO, GxP) with a compliance dashboard and AI gap analysis. Growth unlocks unlimited quality event templates, a developer API and ten edit users.
User reviews line up: quick to roll out, clean interface, one place to point the auditor to. One startup with no prior quality experience describes implementing it to support ISO 13485 and FDA 820 certification. The trade-off shows up later. Reviewers note that complex CAPAs and supplier management can feel thin as you grow, and you'll need a sales call just to see a number.
- Compliance frameworks with AI gap analysis
- Read-only users don't count as edit seats
- Developer API on Growth
- ✓Fastest path to audit-ready in reviews
- ✓Built for small quality teams
- ✓Covers devices, SaMD, pharma and biotech
- ✗No public price, plus an implementation fee
- ✗CAPA and supplier depth can lag as you scale
- ✗Edit seats capped per plan
The easiest eQMS to stand up for a young medtech. If you build hardware with heavy design controls, put Greenlight Guru on the shortlist too.
MedTrainer
MedTrainer isn't an eQMS for manufacturers. It's a compliance platform for care providers: incident reporting, document and policy management, credentialing (with exclusions monitoring and provider enrollment) and a learning library with custom courses and CPR/BLS training. Its pricing page lists hospitals and health systems, ambulatory surgery centers, FQHCs, physician offices, urgent care, behavioral health, dental, home health and labs.
Pricing depends on licensed users, modules and industry content, and the vendor says a quote comes back in 24 to 48 hours. Reviewers rate it 4.3 out of 5 and call it easy to use, if a bit quirky. Its limit for a quality team: it's built around compliance and training, so deep root cause analysis and multi-step CAPA aren't its core. A large health system with a dedicated patient safety office may outgrow it.
- Incident reporting routed to the right people
- Credentialing with exclusions monitoring
- Course library for required staff training
- ✓One tool for incidents, policies and training
- ✓Fast quote turnaround
- ✓Easy for front-line staff
- ✗No public price
- ✗CAPA depth below a dedicated eQMS
- ✗US-focused content
The practical pick for clinics and outpatient groups that want compliance in one place. Large hospitals with a full patient safety team should also look at RLDatix.
SimplerQMS
SimplerQMS is one of the few healthcare eQMS vendors with a number on its pricing page: $17,500 a year minimum, for up to 15 users. That price bundles what others bill separately: implementation, data migration, QMS templates, user training, hosting and validation under GAMP 5. The vendor states compliance with FDA 21 CFR Part 11 and EU Annex 11, ships templates aligned with ISO 13485:2016 and 21 CFR 820, and is itself ISO 13485 certified.
Reviewers rave about customer service. The complaints point to the platform underneath. It runs on M-Files, which can freeze during approvals, and there's no desktop client outside Windows. If your team is on Macs, ask for the demo on one of your own machines.
- GAMP 5 validation included and maintained
- Free Viewer seats for auditors
- ISO 13485 and 21 CFR 820 templates
- ✓Published floor price
- ✓No surprise setup fees
- ✓Top-rated support
- ✗$17,500 a year even for a tiny team
- ✗M-Files base, Windows preferred
- ✗Not designed for care providers
The best pick if you want to know your all-in cost, validation included. For a three-person startup, Qualio or Greenlight Guru will usually start cheaper.
Greenlight Guru
Greenlight Guru only does medical devices, and it shows. The vendor aligns the platform with ISO 13485, the FDA's QMSR (which pulled ISO 13485 into 21 CFR 820 in 2026), ISO 14971 for risk and IEC 62304 for software. Document control, change management, training, CAPA, nonconformances, suppliers, requirements and design controls are linked, so a design change carries through to risk and verification. Every release ships with validation documentation and Part 11 audit trails.
The entry price is public at $12,000 a year, but implementation is quoted on top and the bill grows with each module and seat. That's the number one complaint in reviews. One buyer who picked Qualio instead found it too focused on hardware devices for their needs.
- Design, risk and test traceability
- Validation documentation with each release
- Aligned to ISO 13485, QMSR, ISO 14971, IEC 62304
- ✓Deepest device-specific workflows here
- ✓Published entry price
- ✓Little configuration needed
- ✗Implementation billed on top of $12,000
- ✗Cost climbs with modules and seats
- ✗Less suited to pure software or pharma
The most complete option for hardware device makers. For SaMD or a tight budget, Qualio usually gets you live faster.
Scilife
Scilife, a Belgian vendor, stacks its eQMS in three plans. Essential covers document management, training, competences and KPIs. Core adds CAPA, change control and quality events (deviations, nonconformities, complaints). Core + adds audits, supplier management, risk assessment and equipment. Its industry pages cover pharma, biotech, GDP logistics and medical devices.
Reviewers like the ease of use and the modular setup, with a 4.3 out of 5 average on G2. Complaints mention bugs that worry teams who must validate, slower support at times, and roughly four months to implement. Watch the entry plan: Essential has no CAPA. If you handle deviations from day one, start at Core.
- Stacked plans from documents to audits
- Quality events: deviations and complaints
- Free trial before you commit
- ✓Pay only for the modules you use
- ✓Well-liked interface
- ✓GMP and GDP coverage
- ✗No public price
- ✗No CAPA on the entry plan
- ✗Bugs and slow tickets in some reviews
A sensible step-by-step eQMS for pharma. Go straight to Core if deviations start on day one.
Care provider or manufacturer: two jobs, two kinds of QMS
"Healthcare QMS" covers two purchases with almost nothing in common. Pick the wrong family and you'll pay for software that doesn't speak your auditor's language.
Hospitals and health systems
Your pressure is patient safety and accreditation surveys. You need event reporting a nurse can finish on a phone, investigation workflows, policy management and dashboards for the quality committee. RLDatix is built for this at scale; MedTrainer fits smaller organizations.
Clinics, ASCs, outpatient groups
You rarely have a full-time quality team. You need incidents, policies, credentialing and required training in one place, with little setup. MedTrainer is the closest fit in this comparison.
Medical devices, pharma, biotech
Your auditor is the FDA or a notified body. Since 2026 the FDA's QMSR folds ISO 13485:2016 into 21 CFR 820. You need CAPA, change and design control, Part 11 e-signatures and a validated system, since ISO 13485 requires QMS software validation before use. Qualio, SimplerQMS, Greenlight Guru, Scilife.
- Will the vendor sign a HIPAA business associate agreement if incident reports hold patient data?
- Can a front-line employee file a report in under two minutes from a phone?
- For manufacturers: who writes and runs the validation protocols, and what does it cost?
- Are implementation and document migration in the quote?
- Do auditors and read-only users need a paid seat?
- Can you export every record with its audit trail if you switch?
Also worth a look, without a score
These tools matter in healthcare, but we didn't score them: no readable pricing page, or a buyer profile much narrower than this page. Here's when they deserve a demo.
Also consider
RLDatix: patient safety reporting for large health systems
EnterpriseRLDatix grew out of patient safety event reporting, and its own resource library publishes stories from care providers such as RiverStone Health, which uses its RL6 reporting module. We didn't find a pricing page to read, so we don't score it. Ask for references from a health system your size.

- No public pricing
MasterControl: the heavyweight for large manufacturers
EnterpriseMasterControl serves large life sciences manufacturers, licenses by named user, and offers pre-validated packages for smaller teams. Pricing goes through a sales rep. One startup reviewer passed on it as too involved and expensive for their stage. Keep it for multi-site organizations.
- Best for
- Large manufacturers

- Per named user
Formwork: an eQMS for small SaMD teams
SaMDFormwork comes from OpenRegulatory, a German consultancy, and targets software-as-a-medical-device teams building their QMS with open templates. Our reader was blocked from its pricing page, so we publish no number here. Check the official grid before you budget.

Make your healthcare QMS actually stick
A QMS rarely fails on features. It fails when staff keep reporting on paper, or when nobody budgeted for validation. Here's what works.
Tips
Make reporting faster than paper
If filing an incident takes ten minutes, staff stop filing. Keep the first form to four fields (what, where, when, severity) and make it work on a phone. The quality team fills in the analysis later, not the person on shift.
Start with three processes
Practitioners on the manufacturer side recommend going live with documents, change control and CAPA first, then adding training, suppliers and design. Plan three to nine months from signature to a validated, complete system. A big-bang rollout pushes your first useful date back.
Get validation priced in writing
ISO 13485 requires you to validate QMS software before use and after changes. SimplerQMS includes it; Greenlight Guru ships validation docs with each release. Elsewhere, ask who writes and runs the protocols and how many hours land on your team.
Don't pay for readers
Most people only read an SOP or sign off on training. SimplerQMS gives auditors free Viewer seats, and Qualio separates edit users from everyone else. Count your real authors and approvers before you ask for a quote: it's often half the headcount you first had in mind.
Ask about patient data early
Incident reports often name a patient. If yours do, you need the vendor to sign a business associate agreement under HIPAA. Ask in the first call, not after legal review: an eQMS built for manufacturers wasn't designed around patient records.
Test the export before you sign
Acquisitions, mergers and price hikes force QMS migrations. During the demo, ask the vendor to export a closed CAPA with its full history and signatures. If they hesitate, your audit history is effectively theirs.
Keep exploring
These pages pick up where this one stops: general QMS, manufacturing and documents.
Your questions about healthcare QMS
What is a QMS in healthcare?
It's the set of processes and records that prove you deliver safe, consistent care or products: incident reporting, corrective actions, controlled documents, training and audits. QMS software runs those processes in one system with an audit trail. For a hospital the focus is patient safety; for a manufacturer it's product quality under ISO 13485 or GMP.
Does 21 CFR Part 11 apply to my QMS?
If you keep FDA-required records electronically or sign them electronically, yes. Part 11 sets the criteria for treating those records and signatures as trustworthy as paper. The FDA's guidance reads its scope narrowly, but any device or drug QMS that holds CAPAs and approvals falls inside it.
What changed with the FDA QMSR?
The Quality Management System Regulation took effect in 2026 and incorporates ISO 13485:2016 into 21 CFR 820 by reference. If your QMS already follows ISO 13485, the gap is smaller than before, but the FDA kept some of its own requirements on top. Ask vendors for a QMSR mapping, not just an ISO 13485 one.
What does a "validated" eQMS mean?
It means documented testing shows the software does what you need in your setup. ISO 13485 requires validation of QMS software before use and after changes. Vendors provide part of the package, but the manufacturer stays responsible for the final validation.
How much does a medical device eQMS cost?
The two published prices in our comparison are $12,000 a year for Greenlight Guru, plus implementation, and $17,500 a year for SimplerQMS with setup and validation included for up to 15 users. Everyone else quotes after a call. Compare three-year totals, implementation included.
Can I run ISO 13485 on spreadsheets?
You can, since the standard doesn't require software. But a spreadsheet used for quality records must be validated too, and email approvals are hard to prove. Once you have several approvers and dozens of SOPs, the savings disappear into audit prep time.
Incident vs nonconformance: what's the difference?
An incident or adverse event involves care: a medication error, a fall, a hospital-acquired infection. A nonconformance is a product or process that misses a requirement. Both follow the same loop (report, investigate, correct, verify), but with different forms and vocabulary. Pick the tool that speaks yours.
