What Medform and Zapier do together

Medform is software to run a private clinic, with a CRM side where leads are tracked. With Zapier, leads captured in other tools land there without retyping. The automation is called a Zap: one trigger (the event that starts it), then one or more actions (what it does next), every time that event happens.

On Zapier the app does a single thing, through Create Lead, so the value depends on what you put in front of it. Three results follow. Web form leads in the CRM: a new submission starts the Zap and Create Lead files the person with first name, last name and phone. The source kept on every lead: the Channel ID field records where the lead came from, such as Facebook, your website or Google, so you can tell channels apart later. Only complete leads: a filter in front of the action lets through only the entries that meet your condition.

What Zapier does not do here: Medform has no trigger, so nothing that happens inside it can start a Zap. Every Zap begins in another app. There is no search step either, so a Zap cannot check whether the lead already exists before creating it. Zapier also lists this integration as a beta.

Each Create Lead that succeeds counts as one task, so one lead is one task. An action that fails is not counted, and a few actions take more than one task. Medform is not a premium app, so it works on the free plan: 100 tasks a month, unlimited Zaps of two steps each. A filter in front of the action makes the Zap multi-step, which needs a paid plan. The grid below is how often Zapier checks a polling trigger; it applies to the trigger of the other app if that trigger polls, never to Medform, which has none.

    Open the Apps page

    In your Zapier account, go to Apps and click + Add connection. You need a Medform account, plus access to anything you do not own that the Zap should write to.

    Pick Medform

    Type Medform in the search box of the dialog, select it in the results, then click Add connection. A new tab opens so you can sign in on the Medform side.

    Sign in and grant access

    Sign in to Medform in that tab and grant Zapier access to the account. The connection is saved once, then reused by every Zap you build that creates leads.

    Build the trigger

    Create a Zap, pick your form tool as the app that starts it, and the event that fires on a new submission. Connect the account and choose the form.

    Test the trigger

    Run the test. Zapier brings back the three most recent items, so send a test submission first if nothing comes back, then pick it.

    Add the filter

    From the plus icon, add Filter and set the field to look at, the rule and the value. Use a text rule on a text field. The Zap is now multi-step and needs a paid plan.

    Add Create Lead

    Add Medform with Create Lead. Map First Name, Last name and Phone from the submission, then set Channel ID to the channel that stands for your website.

    Test, then publish

    Test the action and check that the lead shows up in Medform with the right source. Then click Publish. Earlier submissions are not picked up.

When a Medform Zap breaks

Zapier links no help article to Medform and lists no limit of its own, so the cases below are general Zapier limits read against a create-only app.

  • No leads from before the Zap

    A Zap only reacts to what happens after you publish it. Submissions received earlier are not picked up; moving them into Medform is a separate job.
  • A flood of submissions

    If a bulk import suddenly adds many entries to the trigger app, Zapier's flood protection can hold the batch back instead of creating every lead.
  • A missing required field

    Create Lead needs first name, last name, phone and Channel ID. If the trigger app does not collect one of them, map it from somewhere else or fix the source first.
  • Duplicates on the Medform side

    With no search step, the Zap cannot check whether a lead already exists. Check at test what Medform does with the same person twice.

01Is the Medform Zapier integration free?
Yes. Medform is not a premium app, so it works on Zapier's free plan. That plan gives you 100 tasks a month and an unlimited number of Zaps, each limited to two steps: one trigger and one action. A website form that creates a Medform lead for every submission fits within it. Add a filter, to keep only some entries, or a second action, and the Zap becomes multi-step, which needs a paid plan.
02How many tasks does a Medform Zap use?
One task for each Create Lead that completes, and nothing for the trigger. A form that brings in a lead per submission uses one task per lead. Zapier checking the form tool for new entries costs nothing, however often it checks. An action that fails is not counted against your plan. Bear in mind that a few actions on Zapier take more than one task, so check at test what this one costs.
03Is Medform real time on Zapier?
It depends on the other app, because Medform has no trigger. The Zap starts in the tool you pick, and that trigger decides the timing. If it is instant, the app reports the event to Zapier and the Zap starts right away. If it polls, Zapier checks it on a timer set by your plan: 15 minutes on Free, 2 on Professional, 1 on Team and Enterprise. Look at your form tool's trigger before promising anyone a quick reply.
04What is needed to connect Medform to Zapier?
A Medform account, plus read or write access on anything you do not own and want the Zap to touch. In Zapier, open Apps, click + Add connection, search for Medform, click Add connection, sign in in the tab that opens and grant access. Before building, check which channels exist in your account, since Create Lead asks for a Channel ID that stands for the lead's source. Write the channel names down before you start.
05What does the Channel ID field mean in Create Lead?
It is the source of the lead in Medform, for example Facebook, Website or Google. The field is required, so every lead created through Zapier carries a channel. When one Zap feeds one source, you can set the value once in the action. When several sources run through Zaps, build one Zap per source with its own Channel ID, so leads stay sorted by where they came from. Check at test that each lead lands with the right source.
06What if the Medform operation is not on the list?
Then Zapier cannot do it with this app today. The editor offers exactly one operation, Create Lead, with no trigger and no search step. So nothing that happens inside Medform can start a Zap, and a Zap cannot look up a lead before creating one. What you can still do is shape the input: pick the app that starts the Zap and add a filter in front of Create Lead.
07Zapier, Make or n8n for Medform?
Decide by criteria, not by a winner. Availability first: on Zapier Medform offers a single action and is listed as a beta, so check whether the other tool lists it and what it allows there. Then your lead sources: the trigger app does most of the work here, so it must be supported. Finally billing: Zapier counts one task per lead created. Weigh those three points against your own lead volume before you pick a tool.
